• Certification Details
• Vaccines or Positive IGG Titer required
• Annual Fit Test required
• Hepatitis B vaccination or documentation of declination to vaccine required
• Influenza Vaccine required for flu season
• Positive PPD - Negative Chest X-Ray and Annual TB Questionnaire (optional requirement)
• Health Statement or completed physical exam required
• Primary residency verification through Driver's license for compact nurse licenses
• 2 Step PPD OR IGRA (Quantiferon/Tspot) required
• Proof of Varicella immunity or vaccination
• Tdap valid for 10 years from the administered date
• Background check includes Social Security Trace, Employment Verification, OIG Search, and National Sex Offender searches
• EPLS verification
• OFAC check
• Annual skills checklist in specialty required
• 12 Panel Drug Screen within 30 days prior to start
• Education verification
• Ascension onboarding required
• Ascension unit leader to upload specific checklist within a week
• Ascension Onboarding Requirements Education Attestation
• Ascension Nursing Contract Worker Onboarding
• Ascension Nova StatStrip instructions provided
